It failed: the run undid its change and skipped it for the rest of the run, so the graph is still correct, only less optimized. A transform the list holds more than once reads failed at each of its entries. A transform you wrote never reads failed here: its failure ends the run, and optimize returns it instead of a report.

The failure's status name, as Error::code_name() spells it, when failed.
